Your Patient-Centered Medical Home
The Patient-Centered Medical Home (PCMH) model is an approach to delivering high-quality, cost-effective primary care. Using a patient-centered, culturally appropriate, and team-based approach, the PCMH model coordinates patient care across the health system. It helps to build better relationships and promotes communication between patients and their healthcare providers, improve the quality of care patients receive, increase access to clinical and non-clinical resources to patients and improves patient and staff satisfaction while decreasing overall healthcare costs. NEW RIVER HEALTH ASSOCIATION IS A FEDERAL TORT CLAIMS ACT (FTCA) DEEMED HEALTH CENTER
New River Health Association(NRHA) is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
