To be eligible to participate, you must:

• be a female patient of New River Health Association who is between the ages of 21-64, and
• currently be due for cervical cancer screening (pap smear), and
• complete your cervical cancer screening at one of our NRHA locations between the dates of April 1, 2025 – June 30, 2025.

Employees of New River Health Association will not be eligible to win.

If you meet all the above criteria, you will automatically be entered into a drawing to win the basket pictured on the front of this card.

The randomly selected winner will be notified via the primary contact telephone on file in her chart no later than July 15, 2025. Please make sure all contact information in your chart is accurate and up to date at your visit!

No photos or personal information will be shared or posted to social media without the consent of the winner.
